The short term rehab is incompetent.
My father entered short term rehab from a fall for 17 days in [date removed]. In 17 days this facility managed to lose not one but both of his hearing aids. The value of these hearing aids was 12k. When you register the facility requires you to list the patient's property and value of the items. Cypress Village made no effort to safely store my father's hearing aids. Both of the hearing aids were not lost on the same day they were different days. This facility made no effort to learn and prevent the loss of the second one after the first one was lost. Part of my father's cochlear implant was found in the dryer. The other parts and the second hearing aid were never found. I placed signs in his room with pictures of what the hearing aids looked like for the staff to make sure they had not fallen off in the bed prior to changing the bedding. My father was unable to get out of the bed to charge and store his own hearings aids. This facility took no responsibility for this loss.

Thank you for bringing this to our attention. We sincerely apologize and acknowledge that this situation has been deeply frustrating and that our efforts to prevent the loss of your father's hearing aids fell short. While our records indicate that our team made efforts to communicate with you and assist in locating the lost items, we agreed to cover the deductible for the replacement of the hearing aids, and our business office has applied a credit toward the outstanding balance, as we communicated. We also apologize for any perceived lack of responsiveness to your concerns. It was never our intention to ignore your emails or to avoid addressing your grievances. Our team strives to be accessible and supportive to all of our residents and their families, and we regret that this was not your experience. Please know that we are committed to learning from this experience and improving our services. We genuinely care about the well-being of our residents and their families, and we deeply regret that we did not meet your expectations. If there is anything further we can do to assist you, please do not hesitate to reach out. Thank you again for your feedback, and we hope to have the opportunity to restore your trust in our community.

Cypress Village was great. I did not like the memory care unit due to alarms and being a very intense difference from the other floors. Plus they were increasing prices to like $[removed]/month.

If you have the opportunity and desire this is a great community
One thing I really like about the location is the surrounding nature paths, but one thing I do not like about the location is that it is not walking distance to shops so if you want to go shopping you have to call a bus service. Overall, it is a great community.

The word nightmare does not even begin to cover it
I normally do not write many reviews, but I must write one here. My father was just discharged from this facility - he was in the rehab center post hospitalization - and it was nothing short of a nightmare. The level of care and responsiveness of the staff was horrifying. I honestly don't have it in me to go into deep detail, but do NOT send your loved ones here, unless you're fairly indifferent on whether or not you see them breathing again after. This facility was recommended by the Mayo Clinic - we trusted them, not knowing better. Do not make the same mistake. Oh, and his wallet "mysteriously" went missing there. Do not - please - make the same mistake we did and send your loved one here.

Fantastic welcoming experience
Our first impression of this community was great, we really enjoyed how friendly the staff members were and the quality of the apartments, as well as the capability that the whole complex offered. The apartments are spacious, set up in a nice way, and every time someone new moved in they would update the apartment with fresh paint and any other updates it needs. Due to COVID they have limited the activities, but beforehand they had swimming, yoga, dancing, and even had small get togethers

Happy to have my parents at Cypress Village
We chose Cypress Village because it is a continuing care facility, they offered more things on site such as rehab and primary care services. It is also a beautiful campus that is well kept. The rooms are very big for assisted living which was needed since both my parents are living here. They give the extra help and don't charge extra levels of care here so it is good bang for your buck. The food is hit or miss for my parents, they do have a separate dining room for assisted living which is very convenient for them and not overwhelming. My favorite part about the community has been the staff members, everyone I have had contact with has been great.

Misleading care
First hand experience- First Cypress sold the memory care on being able to press a button. Upon moving in this button is located at the front of the door which means your loved one has to walk, crawl or scream to get service. Secondly, the transition was horrible. There is no welcome committee or sit down to go over medications, special routines etc. I actually had to seek out the director to get attention. Third, their system of care is every 3 hours they check on your loved ones. That can be a long time. They are understaffed and as your loved one declines their staff maintain the same level of people therefore do not expect more time to be offered to your loved one. Many people bring private staff in because it can be lonely. if your loved one is not a person who engages in activities then do not expect a lot of personal attention.

Hello there! We have pull cords located in the restroom and bedroom of each Memory Care apartment. When arranging furniture upon move-in, we always recommend positioning the bed near the pull cord for easy access. Our Memory Care program is built on stimulation and socialization, meaning we do our best to keep residents out of their apartments and in communal areas as much as possible. During waking hours, residents are with one another and their care providers participating in activities, exercises and crafts. When residents are in their apartments, care staff round regularly and often to check on them. If you have any further questions or concerns regarding our care and safety practices, please call Robin Kaleel, Memory Care Director at (904) 807-6222.

Skilled Nursing Rehab Nightmare
At first our family believed Cypress Village Assisted Living/Memory Care and Skilled Nursing would be a perfect fit for my parents who are at different levels of care needs. The facility looks impressive but isn’t as great as you may first think. This was our experience. My mother was admitted to their Rehab Center suffering with a variety of issues, including dementia and Urinary Tract infections. After about a week they determined they didn’t want to keep her in Rehab and set to discharge her with a definitive date, with no home to go to or another skilled nursing/Rehab facility lined up. I was traveling out of the country at that time. No other family in Jacksonville. Our whole family was shocked and highly unnerved. I appealed to let her stay for a few more weeks from out of the country as we had also scheduled various tests and specialist appointments at MAYO Clinic in the ensuing weeks. I never got an adequate response other than communicating to other family that they would not extend the discharge date even a few weeks. They reported she was not making sufficient progress in therapy, thus for Medicare reasons, had to discharge her. I believe this was highly misrepresented and is contrary to the 3-4 therapy sessions I personally witnessed right before leaving for my vacation abroad. Other therapists applauded her efforts. Despite her conditions, she was still making strides. Their lack of compassion and reasonableness under the circumstances was ludicrous, unprofessional and insensitive. Thankfully we were immediately able to get her into the Mayo Hospital and then discharged by MAYO to another Rehab. She continues to now make progress under care of a variety of Mayo Specialists who are on a path to change her medications that were determined to be ineffective for several reasons. She’s made great improvement in the last 3 weeks. She’s almost standing alone, walking with a walker and her appetite and mood have improved. How shortsighted Cypress was in not giving our family a reasonable period of time to help her. Do you think a week of therapy is adequate to say Medicare requires us to discharge her since she’s not showing good improvement? How would you feel if this happened to your Mom or Dad? Personally, I wouldn’t send my loved ones to Brookdale Cypress Village. If you do, I caution that this might happen to your loved one.

So glad that I'm here
Cypress Village is terrific and a good fit for me. There are more activities than I could ever take part in and I enjoy the ones I do. There are Sock Hops with a DJ playing 50's music. I have signed up to take a couple of field trips as well. Our main dining area has a community table for the new residents and the long time residents join us so that we socialize with everyone. There is also a bistro and a Fancy dining room that you can pick from. They all 3 have Fabulous food. We also have a resident doctor here 1 day a week and a 24hr wellness clinic. I am so glad that my advisor finally convinced me to come and check Cypress Village what a wonderful service you offer.

  • Confirm the availability of required services: Before you commit, make sure that the community offers all the services your senior needs, and that those services can be provided within your budget.

  • Know the Florida regulations for Assisted Living: Each state has its own laws and inspections processes to ensure seniors reside in a safe environment. Depending on your state, you may even be able to look up inspection reports for the community(ies) you’re considering. Learn more about Florida regulations for Assisted Living.

Cypress Village, a CCRC offers Studio, 1 Bedroom and 2 Bedrooms. Learn more.

A Place for Mom has scored Cypress Village, a CCRC 7 out of 10 using our proprietary review score.

We assign review scores to give a more reliable view into senior living communities and home care agencies. Our review scores prioritize reviews that are recent — the past 24 months — because we know families need current information when choosing senior care.

Those with many recent, positive reviews receive a high review score, while providers with few recent reviews — regardless of how positive — receive a lower review score. Communities with no recent reviews will not have a review score, even if older reviews are positive. The maximum A Place for Mom review score a community can receive is 10 points.
