Very disappointed
My father in law was a hard patient but they evaluated him before they took him in. At one point they said he needed to
Leave because of his language. We understood. He had never talked like that before. We had looked at several places but they wouldn't take him because he couldn't get around well. I notified them every place I checked. I had way in advance a trip to visit my grandkids in New York. When I was there he was admitted to the hospital. They called me there and said he wasn't allowed to come back and that the hospital said they couldn't keep him any longer. When I said I wasn't home, they called my husband. I am the POA. They wanted him out. He was paid till the end of the month which was just a couple of days away. The hospital was calling and I was on my way home. After 24 hours in a train, I had to find a place for him to go and it was a weekend. My life at that point was so stressful and he ended up going to a good place but no one told him he was moving. They just loaded him in an ambulance and delivered him to a new place. Hope never does this to me

Director is responsive to feedback, support staff is not super friendly, nursing staff is mostly friendly, food is not good, but seems to be improving, and the event offerings are impressive for the size of the community

Happy with our choice
Cedar Creek of Warsaw is an intimate facility that has a lot of activities which would be great if your loved one likes to join. My grandmother opts out of those. She enjoys the staff and the routine of having her meals in the dining room.

A great place for loved ones
Loved the family atmosphere. Staff were all very caring and compassionate. Mom was under hospice care while there and they worked very well with the hospice team to coordinate Mom's care. Mom chose not to really participate in any activities so I really can't speak to that question. However, residents had nightly devotions and would have music. Since Mom's room was close to the front, she was able to enjoy the music. I'm very thankful that we found Lake City place for Mom.

I would recommend.For the most part, mother has adjusted and is enjoying it. They seem to be a little understaffed on the weekends when visitors are there but all in all it's pretty good. We picked this community, because my mother wanted to remain in Warsaw.

What we like about Lake City Place are all of the staff, they are friendly with the exception of a few. The staff seems to genuinely care about the happiness/wellbeing of the residents.

Review for Lake City Place
We are very satisfied with everything. They have been very understanding, and kind even though my loved one was not kind at times. My loved one felt at home from the beginning and every time we are there he tells us how good the care is.

Would Not Recommend
I would not recommend Lake City Place to anybody. My loved one's room was hardly cleaned at all the entire time she was there. They had problems keeping housekeeping staff, so they just didn't do it. The riser seat on her toilet had not been cleaned the entire time she was there. There were three staff members there who were very much bullies. When administration was not there they treated the residents awfully. They treated them like children and told them to go to their rooms. One night at dinner, my loved one witnessed another woman at her table not get served a tray. When she spoke up for her, the staff member said the woman didn't turn in an order so she doesn't get anything. And they told my loved one to mind her own business. After my loved one insisted further, they brought the other resident a makeshift meal. But they further told my loved one to mind her own business. When we went to administration about it, the staff member lied and said that didn't happen. Administration then turned on my loved and told her something difficult that went on in the staff member's personal life, which is completely irrelevant. Afterward, they retaliated against her by continually not answering her call button. So she couldn't make it to several meals because nobody would get her up to get her dressed. They also continually messed up her medications and were not open to corrections when I went to them. Again, they retaliated against my loved one in the way they administered her medications from that point onward. I have since moved her elsewhere. She is doing very well there, and they are being very nice to her.

If You Have a Loved One In Need of Memory Care, This is the Place for Them
This is a great community! If your loved one needs assistance with memory care, I would recommend you place your loved one at Lake City Place. The staff know how to deal with memory care residents. The staff are very helpful and knowledgeable towards the family's questions and concerns.
There are three meals a day included and they try to have all the residents come to the dining room. If my loved one doesn't like what is being served, there are other choices for them to choose from. Also they will accommodate resident's requests to eat in their room.
They do allow small pets for the residents here. My loved one loves it and it makes it feel like home.

A good place for mom
I like that this community is all on one floor. It also has access to a courtyard so that people are able to enjoy it without wondering off. You are able to have pets and pets are able to come and visit my loved one. I looked at other places and they were two floors or would have required my mother to more if she were to have any changes and I did not want that. This is an age in place facility and works great for us.

Mom loves it!
My mother is almost 94 and has mild dementia. She isn’t able to live alone any longer. She has been at Lake City Place since August 2020. She is very happy there and tells everyone that it’s a wonderful place to live. She goes to all the activities and says the food is good. They dispense her medication which another well known assisted living facility in Warsaw does not. This was very important to us as she got her meds mixed up before. I’m happy that she adjusted so quickly and is happy there. I don’t have to worry about her but just go and visit and enjoy each other.

Family Community with a Personal Touch
They have been outstanding. It is a smaller community and has a personal touch. It fits my loved one here. There are only about 45 rooms.
My loved one enjoys most of the food. They have a lot of activities to keep the residents active. They have arts and crafts, cooking, games, balloon badminton and more.

Dad and Mom, long time residents
We are pleased overall how Dad and Mom are taken care of. Everyone has been friendly and helpful as far as they able. We have concerns with the continual change in administration especially and quality and number of staff. Meal time seems to have it’s ups and downs as far as quality and quantity but I can only go by what I hear. Several staff members of past and present have gone out of their way to help my folks in one way or another and for those special people we are thankful!.

Home for our mom
One of the most difficult choices in life is to find a new “Home” for a parent who is unable to live in their home safely. A Place for Mom linked us to Lake City Place.
Mom has lived at Lake City Place for over five years. She does call it home. She receives wonderful care from everyone. I as with her for lunch one day and the cook brought her vinegar for her spinach. Even I didn’t know she liked vinegar on spinach!
COVID restrictions were extremely difficult for everyone but more so for residents in assisted living. The staff at Lake City did everything possible to make residents comfortable and active during this time. We were sent daily updates on health of residents. As soon as visits were allowed we were notified.
I recommend Lake City Place to anyone who needs a new home for their parent. Our mother has been been welcomed and cared for by everyone there. She is content and is living the remaining years of her life surrounded by a second family.

I toured Lake City Place and have gotten to know the staff. I was totally impressed with everything. It is a place where your loved ones can really feel at home. It is beautiful, clean, comfortable and they go above and beyond for their residents.

Mom loves it
My 93 year old mother has lived here since [date removed] She has loved it from the very beginning. She tells me all the staff are so nice to her. She has a nice one bedroom apartment. She still gets around well on her own but needs help with her medications. There is a nurse 24/7 here unlike some assisted living facilities who dispense her medications so I don’t need to worry if she has them mixed up. This is a licensed facility that long term care insurance will help pay for.
