No Communication at All
My father just completed a short stay at Forest View for rehab and physical therapy for a broken hip. There was ZERO communication from the staff (social workers, PT, nursing and doctors) with me or my husband for the 3 weeks he was there. We left several messages and never heard from anyone there regarding my father’s progress and all of our calls went unanswered. My father was first admitted to the 4th Floor- avoid this floor at all costs!! The weekend nurse refused to give him a blanket and then threw them at him! They also do not monitor the patients- two nights in a row, an elderly woman with dementia was standing over his bed at 4am. The staff brushed it off. We never received a Care Plan at all. Our only convo w a social worker [name removed] was when she called to say he was being discharged in a day and a half. They just “he met his goals” but should have never been sent home- he can’t walk and fell twice already. No PT or aid was sent home either and they are ghosting us again

The Forestview Rehabilitation and Nursing home in Forest Hills failed to protect New Yorkers most vulnerable and weak. My mother was admitted to this facility with fracture to her spine few days before complete visitor shot down due to COVID-19. During most difficult time instead of supporting, the entire staff was rude not only to patient but to their relatives, who where unable to visit and worrying about loved ones. Staff didn’t care, doctor and nurses were not able to provide truthful information about their patients. My mom when admitted was in good spirits and great mental health, within 2 weeks she deteriorated to the point complete disorientation. Until she started coughing we didn’t not know that this facility infected was ith COVID-19. My mom tested positive and died 2 days after transferring to local hospital. This place is a Hellzone. Avoid this place, just run.

My grandma was admitted to the facility on [date removed] for rehabilitation purposes due to fall and fracture of the the T-10 Vertebrae. The first few days, she was receiving therapy, and my mother was there to advocate her treatments. After a few days, the facility was on lockdown due to COVID-19 and my grandma was left alone with the healthcare providers. A few days later, our family then received an update from our grandmother. She is no longer able to participate in physical therapy. My mom did contact the physical therapy department and the nursing staff about my grandmothers condition but nothing has been changed and my grandma was deteriorated day after day. One day my mom and my aunt attempted to contact someone at the nursing staff or nurse supervisor for over 6 hours but they were unable to reach anybody. In the meantime, my grandma was begging to have water because she was very thirsty but she was ignored because she doesn’t speak English and she wasn’t given proper attention. My grandma was very weak and lost consciousness and every time when my mother called the nursing staff for an update, my mom would get updates saying that my grandma is stable and is asleep. I also would like to state that my mom is a nurse and my aunt is a doctor so they know what they’re talking about. They tried to get the message across to the nursing staff and even the director of nursing that something wrong is happening to my grandmother. My mom went to the facility one day and they weren’t letting her inside to visit my grandmother. She started to beg to talk to the manager/administrator of the facility. An assistant of the administration showed up and had a conversation with my mother. They tried to calm down my mother and say that the nurses are doing the patient care at 3:00 AMand are unable to pick up the phone for any updates to family members. My mom then informed them that she herself is a nurse and knows when and what kind of patient care they can do at 3:00 AM. Then an administrative director called a nursing director. My mom was then begin while crying to test my grandma for COVID-19 because she was very scared and worried that my grandma had it. The director of nursing then told my mother that no one in the facility had corona virus and assured her that my grandmother didn’t either so they didn’t test her and instead just diagnosed my grandma with bronchitis. Two days later, when my family decides to take my grandma home, we received a letter from the facility saying the first patient was diagnosed with COVID-19. Our family is still in the procedure of discharging our grandma from the facility at 11:00 AM. My family then received a phone call the same day at 10:15 AMthat my grandma, in distress, her oxygen level drops to 70% and she will be transferred to the hospital. I also want to point out that this during this situation, my family wasn’t even able to see my grandma. After the transfer to the hospital, my mom and my aunt were at the hospital to which she was transferred to but still not able to see her. Two days later, we got confirmation that my grandma was tested positive for corona virus and was placed on palliative care. We lost my grandma two days later and asking all public to know about it. My point is that they put the whole facility in danger with 165 patients and god knows how many faculty members.

This facility “Forest view rehabilitation nursing home in forest hills NY” is horrible, BE AWARE my mom was there for rehab, which I might add that it was the only thing that’s worth staying in that place . The staff were rude to the patients, and it looked like to me that they did not cate at all about them, this is what I saw from my experience, when it was time for lunch and dinner the staff did not help much into feeding patients that needed. My mom sat in her wheel chair 6 hrs, before asked to be put to bed, misleading web site don’t believe what you see, staff need to be monitored closely by supervisors, or replace to more kind hearted person. STAY AWAY!!!!

My mom was admitted to the Forest View at age 91 after hip replacement surgery and lived there for 6 years. I that place because of kosher food, good location, clean, spacey and well-decorated building with beautiful rooftop patio. The upper floor is fully used for physical therapy, recreation center, concerts, special events. The most precious part of that place is people who took good care of my mom up to her last breath. I am very grateful to those who used their knowledge and warmth of heart to assist, to help. Special thanks to [Removed], assistant director of nursing, [Removed], director of physical therapy, staff of the fifth floor.

I find not enough stimulation for dementia patients. They are in a large room not even facing the tv which is on. The person I visit is sitting in a wheel chair most of the day. If there is an aid in the room they do not engage in any conversation with the residents.

A Caring Environment
My mom ( age 90) was at this facility for 2 months. When she first arrived I thought that she would never be able to walk or be able to leave the nursing home. My mom left about a month ago and she is doing well in an apartment for seniors, walking with a walker and making new friends. The staff is friendly, caring and supportive. The physical therapists encouraged her to walk a little more each day. The nurses and aides were always smiling and helpful. The administrative staff was always available to answer any questions that I had. They were also very helpful to me and were concerned that as a caregiver I was taking care of myself. Today I received a call from [Removed] as a follow-up to find out not just about my mom but how I was doing. They made a difficult time easier for my mom and for me.

Forest View is a good rehab facility. It is supposed to be one of the best. They even have activities here. After a couple of weeks they are doing a good job.
