Experience with Fairview Park
The Legacy Reserve-Memory Care facility is a very welcoming facility for my Mom. She is in a safe place with other residents that she calls "my people." Many daily activities are provided for the residents to participate in.
Her room has her own furniture, pictures and keepsakes and reminds her of her home. She enjoys the meals provided. The staff are knowledgeable, caring and very helpful to the residents as well as their families. I would recommend this community to others.

Overall experience of Memory Care: 3
Services to Memory Care (MC) residents are very basic — what the law requires and rarely more. If not for the tenderness of many of the on-floor caregivers, our overall experience would drop quickly to 2, maybe lower.
As a result of minimal services, the MC unit is tragically understaffed. The staff there when Dad was a resident was for the most part, tender, dedicated, and completely overwhelmed. To give time, attention, or care to one resident is to take away from another. The claim that there are always two staff members on the MC floor is untrue based on our experience. Residents are especially underserved on weekends. Our loved ones are so very vulnerable when we rely on others to care for them, and overall care, meaning safety, nutrition, human dignity, mental stimulation, and having the best day possible given your circumstances was low in our opinion.
Overall experience of Independent Living: 1
The Independent Living facility looks good, but no. We would not recommend it. Here, as in MC, the general attitude is "good enough". They appear to have trouble keeping staff; food, which is the reason we chose this place, is unimaginative; staff sometimes doesn't even show up — which results in buffet-style service. Resident feedback meetings are held, but response is very unsatisfying and residents are routinely reminded that "this is INDEPENDENT living" and they must look out for themselves (even in case of fire). When there's a problem, the response is frequently an excuse, no apology, no plan to fix things.
Cleanliness: 3
It's okay. Like other categories, "good enough" rules. My mother's apartment, which should be cleaned weekly, has regularly
not been cleaned as promised.
In MC, it's difficult to keep up with the various causes of pervasive urine smell. The staff tries to do so but isn't always successful.
Friendliness: 4+
Friendliness is one of the strengths at Legacy at Fairview/Legacy Reserve. Most staff members know the residents by name and also sometimes the family members. We were very impressed by this. Memory Care and Assisted Living front desk personnel (concierges) are flat-out amazing; we were impressed by this factor, and I think now, comforted and encouraged by it.
Quality of Care in Independent Living: 2
The administration doesn't want to be responsible for any kind of "care", and the services promised in the agreements and handbooks (food, cleaning, etc), have been declining steadily and markedly in the last four months.
Quality of Care in Memory Care: Depends on the Shift, overall 2
Individual staff members are amazing; however, their abilities to assist and really provide care is profoundly limited. They do a fairly good job of redirecting residents in their care, but this is not a place where residents thrive. Our loved one did not thrive there. By experience and observation, this MC unit is more of a place to "put" your loved one; there is no special intervention for MC residents that is uniquely designed for their needs. There are some activities, and MC residents can also "share" in the Assisted Living activities, but only if the resident is sufficiently independent so that there's no particular need for supervision, or if he/she is accompanied by a family member. Otherwise, these programs cannot be attended. Again, it's a matter of minimums — what is the least that can be done?
Insufficient staffing is a factor in quality of care because of simple watchfulness: Dad fell often, sometimes within , ,sight of a caregiver who simply could not get to him in time to assist. By "insufficient" we mean for the need at hand, not necessarily in comparison with staffing minimums established under law.
We were profoundly grateful to have a Ring camera set up in Dad's room. On at least two occasions, he had slipped to the floor and went for many hours unnoticed and unchecked-on.
Administrative Staff: 1
And by this, I mean management staff. Corporate goals appear primary. I witnessed, more than once, the regional/area manager park a vehicle blocking the access of other vehicles to the front door of IL and hold up his finger for other cars to continue waiting while he entered and spent more than a few minutes in the building (he did not appear handicapped). When my father was dying, our family was able to reserve the private dining room so that many of us could attend to Dad without overcrowding his room. The regional/area manager came in and congratulated us on our new employment there (mistook us for new employees).
On-site administration is superficial.
Food in Memory Care: 1
Awful. Unimaginative, carb-driven, low in protein, vitamins, and minerals. Dad liked the meatloaf, though. This is not what it seems when you see the printed menu or the administration describes how seriously they take nutrition. Between design and delivery there's a great gap; what is delivered is troubling. We visited Dad almost daily and never witnessed hot food on the table — always lukewarm at best. My dad was a great food lover, but not there. There was no back-up: protein supplements, etc. Staff did not have time to notice eating patterns and make adjustments or make sure that daily requirements were met.
Food in Independent Living: 1
As previously mentioned, food was a primary factor in choosing Legacy at Fairview/Reserve, but it has deteriorated quickly and greatly. Staff turnover and call-ins impacts the food quality and service. Normal service is now frequently by buffet. There are fewer offerings at breakfast and food (even whole fruit) isn't replenished. When a resident asked today for a second yogurt, she was told, "I'm only allowed to give one." My impression is that cost-cutting measures are seriously underway.
Activities in IL: 3
My mother finds these uninteresting and especially the exercise opportunities sadly lacking. Again, there's an appearance that cost- and attention-cutting measures are underway. Also, shade outside the buildings is very limited. Think about that in a climate like this.
Activities in MC: 1
These are uninspired and not particularly directed to the needs of persons with memory issues. Things are scheduled and the staff really tries, but sometimes there's no execution of activities. Activities are time-consumers rather than designed to meet mental or emotional needs. There is no music therapy, apart from performances in Assisted Living; but again, getting residents over into the AL side of the building poses supervision challenges. There really were no signs of activities offered to address needs particular to MC residents. There are calendars and day-sheets, but the offerings are designed more for passing time than engaging residents.
Overall Value for the Money: 1
The company presents itself well, and many visitors will say, "this is one of the good ones!" We desperately wanted to believe that Legacy would do what it said and be what it seemed, and so we chose this company. Because of the tremendous investment and the tremendous hassle of making changes, as well as financial commitments, changing is difficult. Note: there are no discounts, no considerations for circumstances, no adjustments. The "corporate" colors show clearly in this arena.
My father died here. The experience of his passing was peaceful and beautiful — because of hospice care and the individual personalities of caregivers who went out of their way for him and for us as a family. But his day-to-day enjoyment of this place was

Overall Would Recommend
Memory care seems to be a step down from Assisted and definitely from Independent care.
However, the caregiving staff is kind, patient, and good-humored. All of the staff really see the residents, but even so, there is an institutional approach, rather than personalized, in both food and activities.

This was my first "true" facility visit, and they set the bar really high. I truly appreciate the time that [name removed] spent with me- she is an asset to this community for sure. She wasn't pushy, she asked questions about my Mom and provided insight as to how she would fit in this community. She gave me a lot of things to think about and to look for to for as I continue my search for the best place for my Mom.

They could offer better diabetic friendly snacks/desserts/this is the area where my mom has had some issues with service/portion size. We've had no issues with her medication technicians. She has one who is her main technician, and we LOVE her! She definitely loves the patients. The staff have been very prompt with their responses, when we have questions. There is a new protocol with the MOXY pharmacy, no medications can be out sourced, unless it's from the VA. This is a new change this summer, so no issues yet, but just concerned with any emergency care or urgent care, that might need medications. Just a concern.

Need Leadership Change
Things started great when we toured. The guy we toured with was very down to earth and made us feel so welcome and never filled pressured. Then halfway through we learned he was let go. Then we got the other salesperson, she was rude short with us and came off genuinely fake the whole time we worked with her. The ED seems to not have time for any residents and the food is just terrible.

[name removed] is absolutely wonderful! I could not have been any more impressed by how much she truly cares. The facility is great, rooms are very nice too. Everyone was so very friendly and helpful. Unfortunately the food was not always acceptable, other times it could be fine. The front desk staff were great, med techs tried very hard to keep up with a difficult job. Housekeeping just o.k. but I know my standards are probably too high. The maintenance guy was very good but very busy. The craft room woman was such a bright spot in my Mom's day. I would definitely recommend Fairview Park Assisted Living . The laundry person was great too.

We have had a great experience at Fairview. Mom feels safe and secure and her room is perfect for her. It makes it easy to leave each time we visit, knowing she feels safe and secure.

our experience
They were super responsive when my grandfather pressed his button for help. Staff seemed friendly.
He got multiple UTIs there. I think it's because they had him use multiple pad liners to "save money on depends". The last time I asked them to test for a UTI (he was lethargic, sleeping a lot, moody and grumpy) and then 3 days later his PT asked for the same thing. The next day they called me thinking he had a stroke... it was just a UTI that went untreated. The nurse who called me said they never tested, the head nurse said they did... but yet never treated for one. His UTI was so bad at that point he was hospitalized for 2 weeks. They then wouldn't take him back saying he needed too much help. However, now he is back to needing 1-1 assistance for transfer and is using his walker to walk short distances (across a room) at PT. I feel like on paper they show one service, but it's not what you always get. Mentally he wasn't himself there. Unhappy most of the time. He's doing much better since leaving.

What we like the most about Fairview Park is Nurse [name removed] #1 and the Hair person (now retired). They have a solid medication distribution. The hall staff payed attention to our loved. We also have a great relationship with hospice

Our Experience
My loved one was previously a resident at Fairview Park. The administration REALLY didn't care, communicate or try to fix problems. It was terrible. Staff didn't care. Promises were not met. Medications were not given correctly. The food plan was not followed. I removed my loved one pretty quickly.

Experience at Fairview Park
What was shown during the initial meeting with them was not what is actually the case. [Name removed] told us several things that were not exactly true. Examples are they would have the place cleaned before they moved in. It was filthy, I cleaned it myself. The excuse was they didn't know when they are moving in. Well we paid them the money and supplied all the information they needed and told her we would be moving them in by Friday. [Name removed] also said they had a chef from the Hyatt there and that the food was fabulous. Come to find out, there was a young girl, 21 yrs old, cooking and it was not good. She made things so spicy my mother couldn't eat it. I do believe that has calmed down some now. Also, issues with my parents' medication where they weren't getting what they needed, getting stuff they didn't need (like my mother was getting a stool softener when she had diarrhea.) There are a lot of things that just are not right with that place anymore. My parents will be moving out at the end of this month. As far as the staff goes, the med techs are so sweet and kind to my parents. They love them. At this point in time, I probably would not recommend.

I feel I was guided to close on the unit two weeks before the the third quarter started. This allowed them to increase the rent because she signed in the second quarter. At the moment I am not sure if I could recommend this community to others or not.

pleasantly surprised!
The staff who works with the clients 10 to one seem to have a sincere sense of caring. The administration is very willing to work with you to get the patient in the right place based off of what they need.
They moved my loved one from memory care to assisted living because she was bored in the back. She's sort of in between assisted living and memory care. Room sizes are the same and amenities offered are just fine.
