Fairview Nursing, Continuing Healthcare of Toledo, and now White Oak at Divine. The names have changed, but the players remain the same. Staffing issues, a disconnect between the administration and the staff, a lack of supervision of Nurses and STNA's, the use of unsupervised "agency STNA's," lack of "compassionate care," lies and coverups, all led to my mother's death at this facility. While changing my mother's "bedding" an agency aid rolled my mother out of bed onto the concrete floor. To make matters worse, the nurse et al. put her back in bed and assured me she was alert, talking, and uninjured. Further, I was assured by the nurse that they would be checking in on her periodically. I advised them to contact me immediately if there were changes to her vitals. I found her in bed with oxygen in her nose, alone and unresponsive, the next morning. I immediately advised the nurse of her condition and requested 911 be called. I asked the nurse at what time did my mother become unrespo
