DO NOT GO HERE
This facility should be closed down. My father developed a pressure wound that we were never informed about. He allegedly was receiving wound care from a "wound care doctor" but the facility won't tell us the doctor's name and say they "don't know" who this doctor is. Really??!!! In the events leading up to his death, my father called me to say he thought he was having a stroke. The nurse walked by and refused to speak to me on the phone so we called 911. The facility refused to call 911 for us and did not want to transfer him. He was diagnosed as septic (full body infection) with blood pressure of 69/44 in the ER, massive wound infection and required immediate surgery for his wound. He never came out this hospital stay and died in a few short weeks. When we requested his records from this facility, the first response was "Is this for litigation?" That is not a normal response. Keep your family members FAR AWAY from this facility.

If you loved one needs memory care and falls
My Grandma was at Cityview for 3 days ([date removed]) before she broke her leg. She had three falls according to her chart. Yet nothing was said to us. Nothing was done to protect her from falling. No fall mats no bed alarms no checking on her Nothing! They had 2 LVNs on shift for the 2nd floor.

I would Not Consider It
I toured at Cityview. It was very depressing here. I wouldn't even consider it for the care of a loved one. It felt too institutional; too much like a hospital setting. I would not recommend this community.
